Entry record for raffles and door-prize drawings: who bought in, how to reach the winner, and which numbers are theirs when the stub bucket disagrees.

Record ticket number ranges as you sell ('#41–45'), not after — the entry sheet is the tiebreaker when a stub is smudged or missing.

To print: adjust the title, rows, and columns above (column headings are click-to-edit), then hit Print. To save a PDF instead: choose "Save as PDF" as the printer in the print dialog — same result, no software needed. Everything except the sheet is stripped from the printout automatically.
