Printable AA / NA Meeting Sign-In Sheet

Attendance sheet for AA, NA, and other anonymous fellowship meetings. First names or initials only — anonymity is the tradition, so the sheet asks for nothing more.

If attendees need court-verified proof of attendance, that's usually a separate slip the chairperson signs per person — this sheet is the group's own record. Check what format the court accepts.

To print: adjust the title, rows, and columns above (column headings are click-to-edit), then hit Print. To save a PDF instead: choose "Save as PDF" as the printer in the print dialog — same result, no software needed. Everything except the sheet is stripped from the printout automatically.
