Printable Church Visitor Sign-In Sheet

Welcome-table register for church visitors and guests, with an explicit opt-in column so follow-up goes only to people who asked for it.

The "would you like a follow-up?" column matters: contacting only the yes rows gets warmer responses than cold-calling every name.

To print: adjust the title, rows, and columns above (column headings are click-to-edit), then hit Print. To save a PDF instead: choose "Save as PDF" as the printer in the print dialog — same result, no software needed. Everything except the sheet is stripped from the printout automatically.
