Daily attendance sheet for daycare drop-off and pick-up, with initials at both ends of the day. Most state licensing rules require a written record of exactly this: who brought the child, when, and who took them home.

Keep completed sheets — most states require daycare attendance records to be retained for several years, and they're your documentation in any dispute about pick-up.

To print: adjust the title, rows, and columns above (column headings are click-to-edit), then hit Print. To save a PDF instead: choose "Save as PDF" as the printer in the print dialog — same result, no software needed. Everything except the sheet is stripped from the printout automatically.
