Ask visitors to sign in as a safety requirement, not a favor — compliance jumps when it's framed as "the seller asks that all guests register."

To print: adjust the title, rows, and columns above (column headings are click-to-edit), then hit Print. To save a PDF instead: choose "Save as PDF" as the printer in the print dialog — same result, no software needed. Everything except the sheet is stripped from the printout automatically.
