Printable School Pick-Up Sign-Out Sheet

Early-dismissal and end-of-day release log for schools and after-care: which adult took which child, when, with a signature on the line.

Check the adult against the student's authorized pick-up list before they sign, not after — the signature documents the release; the list check prevents the wrong one.

To print: adjust the title, rows, and columns above (column headings are click-to-edit), then hit Print. To save a PDF instead: choose "Save as PDF" as the printer in the print dialog — same result, no software needed. Everything except the sheet is stripped from the printout automatically.
