Printable Patient Sign-In Sheet

Front-desk sign-in for clinics and medical offices. HIPAA permits sign-in sheets as an "incidental disclosure" as long as they don't reveal the reason for the visit.

Never add a "reason for visit" column — that's the line between a permitted sign-in sheet and a HIPAA problem. Name, time, and provider are fine.

To print: adjust the title, rows, and columns above (column headings are click-to-edit), then hit Print. To save a PDF instead: choose "Save as PDF" as the printer in the print dialog — same result, no software needed. Everything except the sheet is stripped from the printout automatically.
