Printable Church Nursery Sign-In Sheet

Check-in sheet for church nurseries and children's ministry, with a pickup code column so children are released only to the adult holding the matching tag.

Add allergy notes next to the child's name at drop-off — the nursery worker who needs that information at 11:40 won't have time to find a parent.

To print: adjust the title, rows, and columns above (column headings are click-to-edit), then hit Print. To save a PDF instead: choose "Save as PDF" as the printer in the print dialog — same result, no software needed. Everything except the sheet is stripped from the printout automatically.
